Summer Camps Physical, a must!

All Scouts and Adults attending one or more of the Summer Camps must obtain a Camp Physical in order to participate.

You must complete Part A and Part B of the BSA health forms and have a Doctor complete Part C.   The form can be downloaded at: https://filestore.scouting.org/filestore/healthsafety/pdf/680-001_abc.pdf

Each Scout and Adult also needs to provide a copy of his immunization history and a copy of his health insurance card. Note – You can provide a copy of your Scout’s immunization history or write out his immunization records in the space provided on Part B.

*** The Troop has arranged for two doctors to provide Summer Camp Physicals FREE OF CHARGE on the evening of Tuesday, May the 1st.  While you are certainly welcome to obtain the required Summer Camp Physical from the doctor of your choice, please contact Scott Wareing by e-mail at Scott@pwbonds.com or via phone at (602) 821-0099 to sign up for one of our open time slots.

We have 48 available time slots between 6:30pm and 8:15pm. As the Troop will be working on the Swimming and Lifesaving Merit Badges down in Landes Hall, Scouts attending Philmont or Emerald Bay are being assigned time slots from 6:30pm to 7:15pm.  Scouts attending Geronimo are being assigned time slots from 7:30pm to 8:15pm. While we are going to give priority to our Scouts, our Adult volunteers are welcome to use any of the remaining time slots after all Scouts who are interested have signed up.

The physicals will take place at Valley Presbyterian Church.  Please check in at your assigned time in Room 415.  Room 415 is on the 2^nd floor above the court yard on the south side of Landes Hall.  If all goes as planned, you should be complete with your physical within 30 minutes.

Six slots are open at each of the following intervals; 6:30pm; 6:45pm, 7:00pm, 7:15pm, 7:30pm, 7:45pm, 8:00pm, and 8:15pm.

Kindly contact Scott Wareing at your earliest convenience to sign up for the physicals as the time slots will be assigned on a first-come, first-served basis.
